Check all aspects of the swing-arm carefully, particularly the pivot area, the suspension linkage and the chain tensioners. The swing-arm should pivot freely (with the shock disconnected obviously) on it's fulcrum but without any sideways play and the tensioners must keep the wheel running true, perfectly perpendicular to the swing arm fulcrum.
If any of the above points are not as they should be then renew the critical parts in question.
