It's a cam chain tensioner, it does exactly what it says on the tin..
If you've got your engine apart it's a false economy to not change this as they never lasted a high mileage when new, and there's a lot of evidence to suggest the original 38 year old tensioner blades can deteriorate and fall apart
Note, you will need to buy the spring and protective sleeve separately.

The horizontal tensioner works as the designers intended (opposite to the vertical one). Some people think that the stock tensioner is needlessly strong and that it overloads the chain though. On the other hand the aftermarket solution costs considerably more. If you go for this tensioner, the manufacturing quality is good and it fits well.
